Submit a Claim - Claim Instructions

To qualify for a payment from the Net Settlement Fund, you must submit a timely and valid Claim Form. The Claim Form must be submitted online no later than May 2, 2019 or, if mailed, postmarked or received no later than May 2, 2019.

You may submit the Proof of Claim Form:
